Subcutaneous Emphysema, Pneumomediastinum, Pneumoperitoneum, and Pneumoretroperitonium: Uncommon Complications of ERCP

چکیده
Context Endoscopic retrograde cholangiopancreatography is a technique used to diagnose and treat certain problems of the biliary or pancreatic ductal systems. It is a safe procedure and serious complications are uncommon. Pneumomediastinum, pneumothorax, pneumoperitoneum and subcutaneous emphysema are rare complications after endoscopic retrograde cholangiopancreatography. Case report An Eighty-year-old woman hospitalized for lower respiratory tract infection was diagnosed with pneumomediastinum, pneumoperitoneum and subcutaneous emphysema after performing an endoscopic retrograde cholangiopancreatography during her hospital stay for increased pancreatic enzymes and dilated common bile duct. With the absence of a perforation site on imaging studies, a conservative treatment was established. The patient was discharged with a very good outcome clinically and radiographically. Discussion This article presents a review of the literature on this rare post endoscopic retrograde cholangiopancreatography complication especially the mechanisms of air leakage in the absence of a perforation site. Conclusion Pneumomediastinum, pneumothorax, pneumoperitoneum and subcutaneous emphysema are rare but serious complications after endoscopic retrograde cholangiopancreatography. Conservative treatment might be appropriate with the absence of a perforation site with a very good outcome. Received July 15th, 2015-Accepted August 28th, 2015

Complications related to endoscopic retrograde cholangiopancreatography (ERCP) include pancreatitis, hemorrhage, cholangitis, and perforation. ERCP-related perforation is uncommon, but mortality rates are high. Diagnosis requires a high clinical suspicion for early detection to allow optimal management of the perforation and a better prognosis. Treatment depends on the location and mechanism an...

Although endoscopic retrograde cholangiopancreatography (ERCP) is an effective procedure for the diagnosis and treatment of the pancreatic and extrahepatic biliary tract diseases, it is still related with several complications. A female patient who underwent an ERCP with sphincterotomy developed massive subcutaneous emphysema along with pneumomediastinum and pneumoperitoneum. Although mild resp...

BACKGROUND The presence of subcutaneous emphysema, pneumomediastinum and pneumoperitoneum simultaneously is a rare complication of upper gastrointestinal endoscopy that usually indicates free perforation to the peritoneal cavity or the retroperitoneal space. CASE PRESENTATION We report an unusual case of a self-limited subcutaneous emphysema, pneumomediastinum and pneumoperitoneum following a...

Retroperitoneal perforation during therapeutic endoscopic retrograde cholangiopancreatography (ERCP) is uncommon and is usually manifested by abdominal pain, fever and leukocytosis. We report the case of a patient with post-ERCP subcutaneous emphysema, pneumomediastinum and pneumothorax treated conservatively. A 79-year-old woman with a diagnosis of choledocholitiasis was referred to our instit...
